The airline operated the flight with a line captain who lacked the necessary training and a first officer who was not cleared for line operations.<\/p>\n<p>The incident came to light after the DGCA reviewed the voluntary report submitted by Air India on July 10. The review, which included an inspection of the airline&#8217;s scheduling practices, revealed numerous regulatory violations and deficiencies.<\/p>\n<p>Subsequently, on July 22, the DGCA issued a show-cause notice to Air India, seeking an explanation for the error.
