PM Health Card: Check eligibility criteria, how to apply

ISLAMABAD: Residents of Pakistan can now easily check their eligibility for treatment under the Prime Minister’s Health Card program.

To verify your eligibility for the program, send your CNIC number through SMS to 8500 or access the official website to input your CNIC number.

Eligibility Criteria

The program provides access to a wide network of hospitals across the country. A complete list of participating hospitals can be found here:

  • Must belong to a low-income family.
  • CNIC must be registered with NADRA.
  • Government employees and individuals traveling abroad are usually not eligible.
  • BISP beneficiaries are also included in the program.

Benefits of the Health Card

  • Up to PKR 50,000 annually for secondary care and PKR 250,000 for tertiary care per family.
  • Free treatment at selected public and private hospitals.
  • Transport allowance of up to PKR 350 during treatment.

Required Documents

  • Original CNIC
  • Health Card (if available)
  • B-Form or marriage certificate for children

Eligible patients are required to pay only 50% of the treatment cost, while the government of Punjab covers the remaining 50%.
